Medical Release Form
The tournament will accept 'generic' medical release forms from your club, as long as they are for the current (2008-2009) soccer year. If your team does not have medical release forms at this time, you should use the tournament form here.
Guest Player
Teams with Guest Players need to submit a guest player form. Please download it here.
